In this step-by-step guide, we’ll walk through how to complete a Provision Form after accepting a sales quote. This form ensures the correct setup of your services and captures essential business, compliance, and administrative information. Completing it accurately helps avoid provisioning delays and sets your organization up for success from day one.
After clicking Accept on your quote, you’ll automatically be taken to the Provision Form. Start at the Account Info section:
Next, complete the required Physical Address fields:
Location name, street address, city, state, country, and ZIP code.
If your mailing and physical address are the same, simply check the box to copy the details automatically.
Provide the information for the main business contact or owner:
Title, first and last name, phone number, and email address.
If the principal is also the majority owner, select Same as Principal to auto-populate.
If there are additional owners, click Add Majority Owner and include:
• Name
• Contact information
• Percentage of ownership
If the principal handles compliance, check Same as Principal.
Otherwise, enter the contact responsible for compliance-related requirements.
These are the individuals who will manage users, settings, and updates within the platform.
Add each admin user with their contact information and role.
Choose the appropriate license or seat type for your users to ensure the correct features and capabilities are provisioned.
Before leaving the page, click Save as Draft. This prevents data loss and allows you to continue later if needed.
